Massachusetts Construction Company Cited by OSHA

Revoli Construction Company, which operates out of Franklin, Massachusetts, was involved in a construction accident which ended in the death of a local police officer in Westfield, Massachusetts last year. According to a federal report, the death could have been avoided if better communication and a better safety plan had been put into place at the construction site by the company. Due to the fatality, the construction company was fined $14,000 by OSHA (Occupational Safety and Health Administration). At the time of the fatality, the company was doing a pipeline project for the city of Westfield. The police officer who died was directing traffic at the site when he was struck by a dump truck which was backing up.

The Massachusetts construction company has been cited by OSHA in the past, including in 2009 and 2008. It also received fines in excess of $115,000 in 2005 from OSHA due to its failure to have cave-in protection for its workers. At that time, it had already been cited nine times earlier by OSHA for failure to provide the cave-in protection. Cave-in protection includes a ladder or other means of escape for construction workers working in trenches deeper than five feet. A sudden collapse of such excavations can result in workers being buried beneath soil and debris before they have an opportunity to escape.
